  • Forbes Georgia: The Importance of Evidence in Decision Making

    In February 2017, CIF director George Gotsadze talked with Forbes Georgia. The interview discusses the role of effective management in health systems and impact of innovations in the field. In the article George speaks about CIF experience serving as HSG Secretariat. Read more in the interview:

    Evidence matters – this is the credo of Dr. George Gotsadze, Director of Curatio International Foundation (CIF), a Tbilisi-based non-governmental research institution, and the Executive Director of Health Systems Global (HSG). Dr. Gotsadze respects evidence-informed policy-making and has been doing a great job with his team in researching healthcare systems globally, and providing evidence-based policy advice to policymakers in various countries. CIF numbers speak for themselves: over the past two decades 130 projects have been implemented, more than 50 scientific articles written, and more than 200 studies undertaken, and these figures are constantly growing.

    The world is undergoing major demographic as well as epidemiological transitions. We’re seeing changing disease patterns from the northern to southern hemispheres, as well as an increasingly aging population, which will pose future challenges. Advances in science and gains in life expectancy throughout the world will mean a significantly higher demand on the global economy to deal with the health issues of our planet. All of this will trigger economic challenges and will demand new thinking, innovations, and approaches,” explained Dr. Gotsadze – in an interview with Forbes back in 2013, without knowing that two years later he would become the Executive Director of Health Systems Global (HSG), which in March 2015 moved its secretariat from Copenhagen to Tbilisi, Georgia.

    HSG is the first international membership organization promoting health systems research and related knowledge translation. It brings together researchers, policy-makers, funders, implementers, civil society and other stakeholders from all over the world. Around 1,900 members of the society work together to create, share and apply the knowledge necessary for strengthening health systems globally.

    From demographic to epidemiological transitions; from the increasingly aging population to advances in science, and gains in life expectancy; from infectious disease outbreaks and violence to hidden epidemics of mental illness and malnutrition; from rapid urbanization to changing disease patterns – these are global, national and local trends that health systems around the world inevitably confront. This means that health systems must be resilient in being able to absorb the shocks and sustain the health gains already achieved, or face the major risk of having decades of investment wiped out. At the same time, health systems are incubators of innovation, which is both an invaluable asset for making people live longer and a huge challenge, since healthcare becomes very expensive. So, only thorough research can evaluate and create new ways in which health systems can better respond to people’s emerging health needs and more effectively solve the healthcare dilemmas directly affecting their lives.

    All these aspects came together at the Fourth Global Symposium in Vancouver, Canada, in one main theme –“Resilient and Responsive Health Systems for a Changing World.” These aspects touched upon such directions as resilience: absorbing shocks and sustaining gains; responsiveness: anticipating change, respecting rights and engaging in politics, and health systems as incubators of innovation. The symposium was attended by more than 2,000 specialists from around the world. This symposium was the fourth of its kind for Health Systems Global after Montreux 2010, Beijing 2012, Cape Town 2014, and the first of its kind for the Curatio International Foundation (CIF) team and its leader, Executive Director of HSG, Dr. George Gotsadze.

    It’s now 2017, so Forbes Georgia asked Dr. Gotsadze if anything has changed in global health since 2013, when he last spoke with Forbes.

  • Barriers and Facilitators to Adherence to Treatment Among Drug Resistant TB Patients in Georgia

    [vc_row][vc_column][vc_column_text]Tuberculosis is a global challenge to public health throughout the world. Poor adherence to treatment remains a significant problem that prevents countries from obtaining high treatment success rates that is essential for health systems to control the epidemic and decrease spread of the disease.

    In 2016 Curatio International Foundation conducted a qualitative study to investigate factors that enhance or hinder treatment adherence among Drug Resistant TB patients (DR-TB) in Georgia. The study revealed different types of factors affecting treatment adherence among DR-TB patients and grouped them into structural, social, personal and health system factors according to the study conceptual framework. The study made it clear that all factors are closely interlinked and mutually influence each other.

    The study provides evidence that may help policy-makers develop effective strategies for improving treatment outcomes among DR-TB patients. The study findings might be helpful for other countries in the region where TB burden is also high.

  • Transition Preparedness Assessment

    Sustainability of national HIV and TB programs gains importance in light of recent changes in the global health landscape when external funders are redirecting resources to poorer states while phasing out from middle-income countries.

    Objectively evaluation of the country transition readiness is a key in the transition process. With this aim in 2015, Curatio International Foundation developed a Transition Preparedness Assessment (TPA) Framework with the Global Fund financial support and piloted in four Eastern European countries Belarus, Bulgaria, Georgia, Ukraine.

    The country Case Studies present findings in a standardized way that enable country stakeholders to prioritize areas that need most attention during transition planning and implementation. In addition, the assessment findings are useful for the donors to guide the country in the transition process. These countries share important similarities that are presented in the Synthesis Report. The three countries case studies are also downloadable below.

  • New Study Findings About Tuberculosis

    Curatio International Foundation together with the Partnership for Research and Action for Health organized a meeting at the National Center for Disease Control and Public Health on 26th of December, where two different study findings were represented. Studies aimed to reveal Referral Delay from Primary Care facilities to specialized TB centers and Health System Factors Affecting Adherence to Tuberculosis (TB) Treatment in Georgia. The project was implemented by financial support of the TDR,  the Special Programme for Research and Training in Tropical Diseases, hosted at the World Health Organization (WHO).

    CIF prezented qualitative research findings about adherence to Drug Resistant TB treatment and led discussion around the findings – what can be done to improve treatment adherence among TB patients, looking through health system lens. The study report will be available by the end of January, 2017.

    CIF inventorised all civil society organizations (CSO) working on Tuberculosis issues in Georgia, with the financial support of Stop TB Partnership through the CFCS round 7.  In the frame of this project TB CSO informational directory has been developed which is available here.

  • Meet CIF in Vancouver at HSR2016

    Curatio International Foundation will be featured at Health Systems Research 4th Symposium to be held in Vancouver, Canada November 14 -18, 2016. CIF experts will discuss several important health system-related issues and share particular knowledge with a global audience. We hope our input in the discussions on global health systems research will be valuable and beneficial to future developments.

    ‘Enhancing Health Systems and Sustaining Public Health Gains While Preparing for the Transition from Donor Funding

    A satellite session will be hosted by The Global Fund and the Curatio International Foundation.

    The session will discuss the changing landscape in development assistance for health-related issues for some countries, which has resulted in reduced or terminated donor funding for critical programs like immunization, TB, HIV,  malaria. The session participants will deliberate over how to sustain the public health gains achieved over the past decades through enhancing health systems during the transition period.

    The session will capture the perspectives of donor representatives, researchers and country stakeholders. Participants include Joseph Kutzin and Susan Sparkes from the World Health Organization, Ajay Tandon from the World Bank, Janis Timberlake from PEPFAR, Deborah Kaliel from USAID, Sara Bennett from Johns Hopkins School of Public Health, Jean-Olivier Schmidt from GIZ, Michael Borowitz from the Global Fund, Donald Pardede from the Ministry of Health of Indonesia, Maureen Lewis from Aceso Global, Tamar Gabunia from CCM Georgia.

    CIF Research Unit Director Ivdity Chikovani will present the Transition Preparedness Assessment Tool, which was developed by CIF to measure a country’s readiness for a smooth transition.

  • Curatio International Foundation: Transition and Sustainability Portfolio

    As middle-income countries experience economic growth and increased government spending potential on healthcare, donors have begun decreasing their support under the assumption those countries have sufficient physical space to support and sustain their health programs.

    Countries in Eastern Europe and Central Asia (EECA), Latin America and the Caribbean (LAC) were the first to face these transitional challenges. In response to the need for a well-planned transition process, CIF with the financial support of The Global Fund to Fight AIDS, TB and Malaria, has developed a Transition Preparedness Assessment (TPA) Framework and Tool. TPA helps countries identify gaps in their health systems and beyond that might hinder the process of sustaining public health goods that were reached with external financial support.

    TPA was conducted in Belarus, Bulgaria, Georgia and Ukraine. This process was funded by The Global Fund to Fight AIDS, TB and Malaria. The findings and recommendations of the TPA were used by countries to develop transition plans (TP). With technical support from CIF, the transition planning process has concluded in Ukraine, and is ongoing in Belarus and Georgia.

    Currently, CIF supports the TPA process in four EECA countries – Armenia, Moldova, Kyrgyzstan and Uzbekistan. CIF is also conducting TPA in Jamaica, Morocco and the Philippines. This time around, the financial support comes from both The Global Fund to Fight AIDS, TB and Malaria, and the UNAIDS Geneva office.

  • Article: Privilege and inclusivity in shaping Global Health agendas

    Health Policy and Planning published an article Privilege and inclusivity in shaping Global Health agendas.  CIF director George Gotsadze co-authors the paper together with Kabir Sheikh, Sara Bennett and Fadi el Jardali.

    The article discusses lack of inclusivity in Global Health and possible actions to promote inclusivity and diversity in the field.

    “Northern voices dominate Global Health discussions. Of recent Lancet Commissions, excluding representatives from international organizations, 70% of commissioners on the Women and Health commission came from the global North, and likewise, 71% of the Health and Climate Change commission, 72% of the Global Surgery commission and 73% of the Global Health commission (Lancet 2016). Only two out of the 16-member Board of Directors of the Consortium of Universities of Global Health come from the global South (CUGH 2016). No current or past president and only one current member of the World Health Summit’s scientific committee is from the global South (WHS 2016). Only one of the 17 advisory board members of the journal Global Health Governance is based in a low/middle income country (LMIC) institution (GHG 2016).

    Only 15% of the world’s population lives in high-income countries. Yet Global Health conferences continue to be dominated by invited Northern speakers and important committees on Global Health composed mainly of Northerners.   • Eastern Europe and Central Asia Regional Sustainability and Transition Coordination Summit 20-21 October, 2016 Vilnius, Lithuania

    In recent years there has been a significant decrease in funding from international and bilateral donors, including The Global Fund (TGF) to support HIV and TB response in middle income countries across Eastern Europe and Central Asia (EECA). This raises concerns about sustainability of national HIV and TB responses, in particular programs targeting key affected populations and delivered by CSO’s. To coordinate efforts of the donors in Region Eurasia Harm Reduction Network (EHRN) with financial support from The Global Fund organized the Eastern Europe and Central Asia Regional Sustainability and Transition Coordination Summit in Vilnius, Lithuania.

    Purpose of the Summit was to coordinate donor (THG, PEPFAR, Open Society Foundations, Robert Carr Networks Fund) and technical partners (UNAIDS, UNDP, UNFPA) attempts to assist countries (especially Civil Society representatives) in transition period planning.

    Curatio International Foundation was invited to present Transition Preparedness Assessment Framework and Tool (TPA) that was already piloted in four EECA countries (Belarus, Bulgaria, Georgia and Ukraine) and is under the implementation in Armenia, Moldova, Kyrgyzstan and Uzbekistan.  Presentation was made by Dr. Ketevan Chkhatarashvili MD, MPH, who leads Transition and Sustainability Portfolio at CIF.
